To register for the 2021-2022 school year, children must be 5 years old on or before December 31, 2021.

Registration forms are available via the links at the bottom of this page.

Please print and complete the Registration form in full, sign in the appropriate areas, and ensure you have completed and attached all the necessary documentation.   If applicable, please attach a copy of your child's most recent report card.

Bussing is provided to eligible  students in the Lord Selkirk School Division.  If bussing is required, please complete the Request for Transportation form. If a bus is not required, please indicate that on the registration form. 

Medical Questionnaire - if a medical concern is identified on the registration form, please complete the medical questionnaire.

School of Choice forms should be completed ONLY if applying to attend a school out of the home school catchment area.

Child in Care forms - should be completed ONLY if child is in the care of CFS.

As outlined by the Department of Records and LSSD, a copy of the child’s Birth Certificate, Proof of Citizenship and parent/legal guardian Proof of Residency must accompany all registrations. Acceptable Proof of residency document:  Government issued photo identification that includes current address.  Manitoba Health is not accepted as proof of residency.  To avoid delays in processing, please forward all required documents with the registration.

It is the responsibility of the parent/guardian to supply all the necessary documentation for school registrations.  This checklist should help you:

  • Proof of Citizenship (if applicable)
  • Parent/Guardian Proof of Residency

Once all required forms and documents are completed for students new to the school in Grades 1 to 6, please call 204-338-7510 to arrange a registration appointment with Administration.

Students new to Kindergarten will receive an orientation and Welcome to Kindergarten information at a later date.
